To find individuals functioning capabilities, to identify their needs, and to determine what services are required to assist them in attaining and maintaining optimum independence.
Our assessors will develop Care Plans, make referrals, and arrange for Service Delivery Schedules according to the individuals’ wishes.
Our target consumers include adults or seniors who are physically frail, home bound, intellectual or physically disabled, and those who are on the process of recovery.
